Dr. Polyak has been my PMD for almost 30 years ! She's the best n the only Dr. I trust w/ my care/health ! The rest of the office staff are awesome too. Very pleasant n helpful! The whole group is outstanding !! I'm very lucky to have Dr. Polyak as my PMD ! ?she's smart , compassionate, listens , informative.

Told me I could only complain about three things. That's not always possible with diabetes and all the other problems. Doesn't refer me to a MD she knows, just gives me a phone number of a group of doctors. The doctors in Northern Illinois for instance. My phone could do that.
We're taking 2 new problems under control. I have confidence in Dr. Polyak.
