She may be getting overworked as she is booked a year in advance. When told of my insurance change she seemed surprised that her services were being billed as diagnostic and not preventative as we are trying to prevent me getting skin cancer in the future by checking regularly.

why is so hard to get a year out appointment,

The care provider is excellent, but it takes months up to a year or more to get an appointment with her. more doctors are needed so that patients can be seen in a more timely way.
