The care provided by the Doctor and his assistant was terrific. The only complaint is that Advocate can't seem to grasp that I satisfied my deductible and out of pocket expense limit 3 months ago. Even after 4-5 phone calls and discussions with the staff, I still get asked for a co-pay...Please figure it out, I am done and not paying another dime!

My visits with Dr. Kulczycki are always comfortable. I can talk to him about anything health related and he gives me answers I understand. He talks to me, not at me. He has a great sense of humor. He has been our doctor 19+ years.

Thankful to have a good doctor like Dr. Kulczycki. He's straightforward and knowledgeable and very friendly and personable so I would recommend him to anyone.

Dr. Kulczycki helped lower my cholesterol level (in comparison with my physical one year ago) and he also councils me on some of my other medical needs outside his purview.

Love n respect Dr and staff! Don't like after visit . Hate , Hate, Hate you LiveWell. Takes 3-4 times to log in even tho I verify each letter. Then when finally in, I have to get code. When I get code, I get removed from my sign in and have to start all over! I WANT MY LAB RESULTS!
