There was a new form that NONE of the staff we aware of. Even though my lubricating shot was pre-approved Aurora wanted me to sign a form if the $1200 wasn't covered I was responsible to pay for it!! Really for someone on disability. I ended up getting a cortisone shot instead for this visit.
