This is no reflection on the provider but on the system in place for signing release forms prior to surgical procedures. It would have been nice to have this provided prior my surgery date. But the time I looked at this I'd taken two anti-anxiety meds and been through two rounds of Mohs procedures. I have no regrets just wish I'd seen this ahead of time.

This was a plastic surgery visit for Botox and fillers. So not a medical visit where we would discuss diet and excercise
