As my primary care provider I expected that Dr. Lopez would review and sign a standard fitness for scuba diving medical form. The form requires a doctor's evaluation of whether my prescription medications or past history of a hernia restrict me from diving. As an instructor, I know this evaluation is typically provided by a primary care professional. Dr. Lopez refused to sign, and instead referred me Occupational Health for an appointment to have the form reviewed.
