Dr Perlow is the best.

Having to fill out my health survey on paper and online is overkill. I realize that some people are not comfortable doing it online. Perhaps if you make mention that if you do it online, you do not have to do it with the paper copy or vice versa. One of those copies should be sufficient.

Love all my doctors and nurses. Do not care for all the notes and letters from "Advocate Medical." They are computer generated and do not specifically apply to my health situation. I would appreciate receiving fewer of those. For instance, a lengthy letter about scheduling a wellness visit, when my appointment was only 4 days away. Letter was not necessary.

Dr. Bruce Perlow has been my Primary Care doctor for almost 30 years. I am blessed to have him as my doctor!
