I was astounded to look at my after visit summary and see that it said absolutely nothing about why I had seen the doctor. It was a 6 month check up but I had 3 specific concerns that were discussed, but nothing included in the summary or notes. Why bother with all those endless sheets of past conditions, rx, etc and then leave off the actual reason for the visit FROM THE PATIENT'S POINT OF VIEW? What a waste of paper!
